Around 12:20 p.m., a Springfield Masonic van was traveling west on US-40 when it made a left turn onto US-68 southbound, according to the Ohio State Highway Patrol (OSHP).
A semi-truck was traveling eastbound on US-40 when it failed to yield for a red light and hit the van.
