An early investigation found that a semi-trailer was going westbound on Plymouth Road when the driver went through the intersection, and a southbound vehicle on SR-72 hit the semi near the rear axle, according to Lt. Joseph Weeks with OSP.
The semi failed to yield the right of way from the stop sign at the intersection.
The driver of the vehicle, which Weeks described as a pickup truck pulling a gooseneck trailer, died from their injuries.
The Ohio State Highway Patrol Xenia post has since identified the driver as Justin Mullikin, 36, of Jamestown.
The semi driver, a 45-year-old Jamestown man, refused treatment on scene.
The semi was carrying a full load of grain, which spilled onto the roadway.
