Ismahan Abdikadir Nagati, 26, of Columbus, was killed in the crash, according to the spokesperson.
A preliminary investigation found that a Kia Rio, driven by Nagati, was disabled in the left lane of U.S. 33 when it was hit from behind by a Mack dump truck.
A 60-year-old DeGraff man was driving the dump truck. The spokesperson said he failed to stop for the disabled vehicle.
The impact pushed the Kia into the right lane and it was hit by an International tractor-trailer.
A 69-year-old Cardington man was driving the semi truck, the spokesperson said.
The dump truck overturned and spilled a load of gravel onto the highway before traveling off the right side of the road.
The semi truck and the Kia also traveled off the right side of the road, but the semi overturned.
Nagati was pronounced dead on scene and the driver of the dump truck was flown to an area hospital in serious condition, according to the spokesperson.
The driver of the semi truck was hospitalized with minor injuries.
Both eastbound lanes of U.S. 33 remain closed as crews clear the scene.
This crash remains under investigation by the Ohio State Highway Patrol.
